Before your appointment please complete the following steps:
Step 1: If you provided an email address when making your appointment, then you will receive an email with directions on how to access your patient portal on EMA. Please complete the information about your past medical history, past surgical history, medications, family history, and enter the pharmacy you would like us to send your prescriptions.

If you don't have access to EMA, please print and complete all forms below and bring them with you to your appointment.

Step 3: For some specific conditions, we need a little extra information to treat you best. If you are visiting us for any of the reasons listed below, please click the link and fill out any corresponding forms.
Step 4: How to reach us. We are located at St. Vincent's West Campus next to the Promenade of west Little Rock. We are located off Chenal Parkway.
If you are driving West on Chenal Parkway, you will pass the UPS Store and Domino's Pizza on your right. You will pass Perfect Vision on your left. At the next red light, you will turn left onto St. Vincent Way.
If you are driving East on Chenal Parkway, you will pass the shops of the Promenade on your right and McDonald's. At the next red light, you will turn right onto St. Vincent Way.
The St. Vincent West Campus is located on the left as you drive down St. Vincent Way from Chenal Parkway. The entrance is posted below. We are located in Pavillion 2 (the building on the LEFT, with Cornerstone Pharmacy) as you pull into the St. Vincent West Campus. Our office is located on the 3rd floor.
